PCOS and Hirsutism

Polycystic Ovary Syndrome (PCOS) is one of the most common hormonal disorders affecting women of reproductive age. Among its many symptoms, hirsutism—excess hair growth on the face, chest, or back—is often one of the most distressing.

At Complete Healthcare in Richland, WA, we offer several treatment options for PCOS-related hirsutism, including spironolactone, a medication with proven benefits in reducing unwanted hair growth.

What Is Spironolactone?

Spironolactone is a potassium-sparing diuretic traditionally used to treat high blood pressure and fluid retention. However, it also has anti-androgenic properties, meaning it can block the effects of male hormones (androgens) like testosterone. In women with PCOS, elevated androgen levels are largely responsible for hirsutism, acne, and irregular periods. By limiting the impact of androgens, spironolactone can help reduce these symptoms.

How Spironolactone Helps With Hirsutism

For patients with PCOS, spironolactone can:

  • Reduce hair growth on the face, chest, abdomen, and thighs

  • Slow the progression of new terminal hair formation

  • Improve skin texture by addressing acne

  • Enhance overall hormonal balance when used alongside birth control pills

Spironolactone does not remove existing hair, but it weakens hair follicles and slows future growth. Visible improvement is often seen after 3 to 6 months of consistent use.

Benefits of Spironolactone in PCOS

  1. Effective Symptom Control Clinical studies show that up to 70–80% of women with hirsutism see significant improvement with spironolactone.

  2. Dual Benefit With Acne Because excess androgens also contribute to adult acne, spironolactone often helps improve breakouts—especially along the jawline and chin.

  3. Non-invasive and Oral Unlike laser hair removal or topical creams, spironolactone is taken by mouth and does not require frequent office visits.

  4. Safe Long-Term Option With proper monitoring, spironolactone can be used long-term to manage androgen excess.

Important Considerations and Risks

As with any medication, spironolactone is not without risks or side effects. At Complete Healthcare, we review each patient’s medical history to determine if spironolactone is an appropriate choice.

Common Side Effects Include:

  • Irregular periods (especially if not paired with hormonal birth control)

  • Breast tenderness

  • Fatigue or dizziness

  • Frequent urination

  • Increased potassium levels, which may lead to heart palpitations or muscle weakness if not monitored

Because spironolactone can affect electrolyte balance, we regularly monitor serum potassium and kidney function via blood tests.

Important Safety Warnings

  • Pregnancy Risk: Spironolactone is not safe during pregnancy as it may interfere with fetal development. Patients must use reliable contraception while on the medication.

  • Breastfeeding: The safety of spironolactone during lactation is not well established. Alternative treatments may be considered during this time.

  • Drug Interactions: It can interact with other blood pressure medications, NSAIDs, and potassium supplements.

Who Should Not Take Spironolactone?

Spironolactone may not be appropriate for women with:

  • Kidney disease

  • High potassium levels

  • Uncontrolled menstrual irregularities

  • Plans to conceive soon

In some cases, birth control pills may be used as first-line treatment or in combination with spironolactone for synergistic effects and cycle regulation.
